My quote is indicative until final scope is defined. I think you should consider a html web application with a web backend. This can be provided by a relatively low cost Office 365 subscription (which has other organisational benefits) or by Microsoft Azure.

I can demo an example of a tablet mobile app in this context to capture data and administrative capture of the resulting captured data.
