Build a form as per attached image in php. Display data from a mock up mysqltable.

  1. The form must be display exactly as in the image including all css
  2. The data displayed is only for test, but the php to retrieve and display the data must be in the code.
  3. None of the buttons need to do anything at this point.

All of the bids so far are from people not rated which makes me nervous. This is the first of many pieces of work. So as I cannot work out how to communicate with each of you on this site can you provide me with links of similar work done. Thanks. Kevin

Hi Kevin,

I am available to build this now.

I'm based near Dee Why, Sydney.

I've been in IT for over 20 years and worked in varied roles from developer through to manager recently. I have a Computer Science degree.

In November 2015 I embraced a switch to try out freelance work on the web. The technology stacks I have experience in are Microsoft based however I've recently started using Wordpress, PHP, MySql and whatever else I need to get the job done.

To date my portfolio of work is limited as I've only done 1 job at Upwork [so far].

For reference to this type of work, my personal site is largely built in PHP and retrieves news/article information from MySQL.
