Base on your description I understood you are after responsive and device compatible web portal and/ or APP, which should have several "approval workflows ", export as PDF , print web form (in A4 size) and digital signature feature. Also, you need several reports for analysis (I have created my own reporting web-base tool, which could help you to save your time in reporting portion). Rohan, I just completed a web/mobile application for Agriculture industry here in Melbourne, within which we have changed their information gathering process(forms) into an online system, wherein the output of the software is to generate pdf's for the information collected from users.

It also included login/registration and lot of other functionality.

• Utilize RESTful web services for communication between web front-end and server

1. Users, i.e. either consultant, can login to portal and create a new client's form submission
2. Pop-up box for capturing signature of client if needed

I have evaluated the requirements and its similar to another system I've worked on which is slightly more advanced. It had the following capabilities:-

• Ability to Upload any PDF with fields
• Ability to assign form fields to PDF fields as an Admin function (This way if your pdf form changes you dont need to contact the software vendor so long as your data model is consistent)
• Ability to delcare a signature hot zone and accept signatures.
