convert paper/pdf forms into forms that can be filled out in the office on computer or iPad or emailed back to our office via our website set up adobe acrobat central on a server in the office and teach myself and staff how to use it (so onsite visit may be needed postcode 2217) be available for email or phone troubleshooting attached are the type of forms that I need to be converted.
Need more info but about 5 days on the work you have stated.

Is the existing form on paper is the same as the forms that you attached with this job? If it is, to make it easier, i recommend a minor change to paper form so that scanning it will easier, when patients fill out the paper form. Please Note: My rate is charge per day.
